Waivers

NICA Release of Liability & Participation Waiver

We encourage you to File Online and email to coach OR you can print off the form and hand to the coach.  

  • Prospective Students for Demo Day: A parent/guardian will need to complete the NICA participation waiver. This grants you access to ONE practice session.

  • Go to https://app.hellosign.com/s/GQnZkjN8
  • Enter your email address and click Verify
  • You will receive an email in a few minutes. If you did not get the email, please check your Spam folder
  • Click the Review Document button in the email
  • You will see the Participation Agreement. Click Get Started
  • For students, please enter the full name in the Volunteers Full Name Field.  
  • Our Team Name is Adams County Composite and our League is  PICL 
  • Once completed, you will see a success message telling you to check your Inbox for a copy of the agreement.
  • When you get the email, click on the View Signed Document 
  • Download the agreement (will generate a PDF).
  • Please forward the PDF agreement to Team Director Michael Connelly ([email protected]) so we have a copy on file.
  • That's it - You are ready to roll!
